On Friday at approximately 1:16 p.m., a motor vehicle collision occurred along Pi`ilani Highway.
Maui Police Department officials say the collision occurred as a black 2003 Independence motorcycle was traveling south on Pi`ilani Highway and collided with a gold 2003 Chrysler Caravan that failed to yield while making a left turn onto Pi`ilani Highway from East Welakahao Road.
The operator of the motorcycle was not wearing a helmet and sustained fatal injuries. He was pronounced dead at the scene.
The passenger of the motorcycle was also not wearing a helmet sustained critical life-threatening injuries. She was transported to the Maui Memorial Medical Center for treatment.
Police say the operator of the Chrysler Caravan was wearing her seatbelt and also sustained fatal injuries. She was pronounced dead at the scene.
The front seat passenger of the Chrysler Caravan was not wearing his seatbelt and sustained injuries. He was transported to the Maui Memorial Medical Center in stable condition.
The operator of the motorcycle has since been identified, following the notification of next of kin, as 42-year-old William Kerr Chapman of Kihei.

The involvement of speed, drugs, and alcohol are still pending as the investigation is ongoing.
This is Maui County’s 9th traffic fatality for 2018 as compared to 4 the same time last year.
